REQUEST FOR BAPTISMAL, CONFIRMATION, AND MARRIAGE CERTIFICATE through these steps:

STEP 2: Fill up the form and click SUBMIT.
Make sure to submit complete and correct details and documents.
STEP 3: Parish Admin (Shrine Office) processes the request. If there are no discrepancies, the church office will release the certificate. If not, proceed to steps to request partial changes.
STEP 4: Shrine Office to release requested certificate. Requesting party submits original documents to the Shrine Office for releasing.
